Coordination and Care works with you to understand your NDIS plan. Our support assists you to build the skills you need to understand, implement, and use your plan to achieve your goals. We work with you to improve your ability to connect with the community through informal and funded support networks.

At Coordination and Care we understand that paperwork and forms as sometimes confusing and hard to follow. Coordination and Care provide you with expert help to walk you through the NDIS application process. This can as little as just an answer any of your question, or fully facilitating your application. We make sure that all the required paperwork is submitted to give you the best opportunity to gain funding.

Coordination and Care charges $1000 for our NDIS application support service.

Yes, the Coordination and Care fee is funded in your NDIS plan from your core support funding.

If your application for NDIS funding is declined, Coordination and Care will complete a review to ascertain if there is any other opportunity to resubmit an application. If there is no NDIS funding, there is no charge for Coordination and Care  work in facilitating your application.  

Firstly, congratulation as you now have NDIS funding to work towards achieving your goals. You then need to select a support coordinator to help you put your plan into action. At Coordination and Care we believe we are best placed to help you get the most from your funding as we have been with you throughout the application process of you plan.

No. Choice and control remains with you regarding your funding and how you want to implement your funded services. We believe we are best placed to support you to achieve your goals as we have worked with you during your NDIS application process. 

Firstly, congratulations as at least you have NDIS funding to support you to achieve your goals. There are other ways for you to make you plan work for you. You can use a plan manager to help with some of complexity regarding rolling out your plan. Please talk to us as there are many ways forward.

• There is a focus on developing our internal capability aligned to both the needs of key service providers and NDIS participants based on best practice systems of work. Our goals are simple, to be an enabler for individuals in marginalised groups to improve their potential to achieve their goals supported by the NDIS.

The NDIS measures each case individually and therefore there is no clear eligibility checklist. However, people who experience a disability because of their mental health, and are likely to be eligible for individualised NDIS support will:

• Have lifelong or permanent needs in relation to their mental health or disability
• Require support to participate in activities or carry out everyday tasks

Carers play an important role in supplying needed information during a planning meeting to ensure that services best meet a person’s needs and goals. At Coordination and Care we can provide support for people and carers that promotes, upholds and respects individuals’ rights to freedom of expression, self-determination and decision make.

All information supplied to Coordination and Care is treated as confidential. All information is kept as required by government regulation
